Summary: 

The PRN Registered Nurse will support a team of clinicians in delivering comprehensive serious illness services. The PRN Registered Nurse will be working collaboratively as part of an innovative team building new clinical pathways necessary to grow a new serious illness organization serving SC. This role is instrumental in maintaining a positive culture of high performance and exemplary patient care. This is achieved by a focus on quality and service excellence.   • Performing initial and ongoing assessments of patients/family/caregiver with a holistic perspective that incorporates identification of support systems and barriers to care. 
  • Managing components of illness/disease progression. 
  • Addressing and managing distressing symptoms utilizing both pharmacological and non- pharmacological interventions in collaboration with interdisciplinary team (State of the art pain and symptom management). 
  • Providing nursing education and training regarding symptom management, disease process and advance care planning within scope of practice. 
  • Developing and implementing an individualized plan of care focused on patient/family goals. 
  • Effectively collaborating with interdisciplinary team to meet needs of patient/family. 
  • Identifying and contributing to patient safety initiatives. 
  • Managing & communicating patient/family expectations.  
  • Evaluating and monitoring primary caregiver confidence and ability to provide care.   
  • Conducting appropriate goals of care conversations with patients/family. 
  • Demonstrating comfort level with difficult conversations. 
  • Understanding concepts of grief and loss. 
  • Assessing, identifying, and closing clinical and non-clinical gaps in patient care, engaging members of the interdisciplinary team as appropriate. 
  • Delivering culturally competent/appropriate care demonstrating acceptance and understanding/desire to understand clients from various socioeconomic, ethnic, cultural, and educational backgrounds. 
  • Participating in the creation of mature, healthy teams to achieve excellence in teamwork.  
  • Being a good steward of resources.  
  • Completing accurate and timely documentation. 
  • Ensuring service and quality standards are met to promote patient/family satisfaction. 
  • Utilizing the 5 Gems framework to positively impact CAHPS scores. 
  • Providing spectacular customer service for patients, families, and referral sources. 
  • Ensuring all clinical policies and procedures are followed to promote excellence in clinical practice.

Responsibilities:

  • Knowledge of federal, state, and local regulations r/t Hospice. 
  • Knowledge of Medicare Conditions of Participation for Hospice. 
  • Knowledge of quality standards and reporting practices for hospice and palliative care. 
  • Knowledge of Adult learning & training methodologies. 
  • Knowledge of Philosophy of comfort, dignity, autonomy, quality of life, and empowerment. 
  • Knowledge of common grief responses. 
  • Knowledge of the Big 5 clinical expenses (labor, supplies, pharmacy, DME and mileage). 
  • Practices within the scope of license, certification, or registration, where applicable, and as required by law and regulation. 
  • Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing centers, assisted living communities, etc. 
  • Practices safety and infection control methods. 
  • Ability to challenge processes that are inefficient/ineffective while contributing to the solution. 
  • Proficient in Microsoft Office (excel, word, outlook, power point) & Zoom / TEAMS. 
  • Excellent problem-solving skills. 
  • Strong organizational skills and ability prioritize tasks, detail oriented and flexible. 
  • Excellent oral/written skills. 
  • Excellent interpersonal skills. 
  • Convey warmth, confidence, and competence.

Qualifications: 

  • Registered Nurse.
  • Prior home health or hospice experience a plus. 
  • Active Registered Nurse license in the state of SC. 
  • Valid driver’s license and proof of automobile insurance (or other mode of personal transportation).
  • Prior healthcare experience serving chronically ill patients preferred. 
  • Other duties as assigned.
