The Hospice Chaplain provides spiritual support and spiritual counseling to patients and families in keeping with the patient/family’s belief system. The Chaplain serves as a liaison between patient, family, and their clergy. Provides information and support to the hospice team relative to the patient & family’s spiritual needs and instructs staff how best to support those needs, if necessary. The Chaplain interacts with other community chaplains of different beliefs and background.

Qualifications

  • Trained in spiritual care by an accepted religious organization, graduate of accredited seminary or school of theology, or appropriate certification in hospital or pastoral ministry. Ordained or credential in religious organization of choice.
  • Preferred a minimum of one (1) year experience in counseling in death and dying.
  • Requires intense listening skills.
  • Driving or other mode of transportation is required as work is mostly in patients’ homes which are not usually wheelchair accessible.
  • Must have a valid driver’s license, with available and dependable transportation, and proof of current automobile liability insurance.
  • Excellent verbal and written communication skills.

Responsibilities

  • Provides direct spiritual support to patients, families, and staff.
  • Implements hospice information visits and sign up visits as directed.
  • Assesses patient and family for spiritual needs and concerns within 6 days of admission.
  • Participates in developing the full IDT plan of care within 5 days of admission.
  • Coordinates spiritual care to meet needs and concerns of the patient and family in a manner that is meaningful to them.
  • Contacts, if requested by patient/family, the patient's clergy of choice and advises him/her on admission.
  • Invites patient/family religious representative to attend the IDT meetings.
  • Acts as liaison where needed between hospice team and patient/family's religious organization.
  • Participates in weekly interdisciplinary meetings by exploring and assessing the potential spiritual needs of patients and families, and reporting on services as indicated.
  • Visits hospice patients/families where directed by the IDT.
  • Maintains accurate, legible records of visits to patient/families and turns in paperwork within the time frame of twenty-four (24) hours (next working day).
  • Serves as spiritual resource to staff and volunteers.
  • Responds to patient/family needs after hours when requested.
  • Conducts or coordinates baptisms, weddings, funerals, and other religious rites as requested by patient or family.
  • Participates in continuing education for hospice staff.
  • Follows Agency protocol regarding outside speaking and professional affiliations, clearing all requests through Administrator.
  • Carries out all other duties as assigned.

Functional Abilities

  • Knowledge and sound understanding of hospice philosophy and its concepts of care.
  • Knowledge of and ability to apply spiritual counseling techniques in a wide variety of hospice situations.
  • Ability to work comfortably in a non-denominational or multi-denominational environment.
  • Ability to apply spiritual truths in an affirming and comforting manner.
  • Ability to respond and function appropriately in crisis situations.
  • Knowledge of various religious approaches to and beliefs concerning death and dying.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Ability to travel.
Physical Requirements
  • Prolonged periods sitting at a desk and working on a computer.
  • Requires good hearing to assess situations quickly and respond appropriately.
  • Light to moderate physical activity required.
  • Must be able to lift up to 15 pounds at a time.
