Job Summary:

The Care Coordinator (CC)-Madera is responsible for providing in person crisis intervention, coordination supports, training and education to DCS staff, and intakes to children awaiting an appropriate placement and/or treatment at the Department of Child Safety's (DCS) Madera Welcome Center. The Care Coordinator will maintain accurate and timely clinical service records.


Essential Duties:

  • Demonstrates ability to maintain confidential/sensitive information.
  • Provides behavioral health support to the youth visiting the DCS Madera Welcome Center experiencing either DCS removal or placement transitions and is driven by the child's strengths and unique needs.
  • Provides community-based services to support immediate crisis.
  • Interfaces with DCS and other partners, including DCS placements, regarding child well-being, stability, and case planning.
  • Provides positive, pro-active communication to other system partners related to care coordination needs to increase a successful transition from the Madera Welcome Center.
  • Provides complete documentation that accurately reflects services delivered in accordance with regulatory, contractual and agency requirements.
  • Provides training/guidance to the Madera DCS staff to help support their own engagement of youth at the Madera Welcome Center.
  • Conducts intake by completing all required clinical documentation to provide an accurate assessment of the child's current functioning.
  • Performs other duties as assigned which are consistent with the general responsibilities of the position.
  • Requires advance knowledge of Microsoft Office Suite computer applications, aptitude for learning computer software applications and the ability to instruct others in the use of various software applications.

Requirements

  • Must be 21 years of age (licensing requirement).
  • Bachelor's degree in the field of social work, psychology, or related subject area preferred.
  • HS Diploma or Associate's Degree may be considered with relevant years of behavioral health experience that comply with Office of Behavioral Health Services staff qualification.
  • Behavioral health work experience may be paid, volunteer, or internship experience working with persons with a mental health diagnosis, emotional impairment, or at-risk categories.
  • Proof of COVID immunization.
  • Behavioral health work experience may be paid, volunteer, or internship experience working with persons with a mental health diagnosis, emotional impairment, or at-risk categories.
  • CPR/ First Aid certification required.
  • Must be able to provide own transportation to and from client's homes and/or other relevant organizations or agencies.
  • Position requires variable schedules Monday through Friday 10:00am-7:00pm and may include some evening and weekend hours.
  • Bilingual in English and Spanish preferred, but not required.
  • Must possess or be able to qualify for a valid Arizona department of public safety level one fingerprint clearance card.
  • Must possess a valid Arizona driver's license and be insurable under the agency's automobile policy.
  • Must be able to meet training and agency requirements for the position.
