Position Summary:
JCCA is seeking Case Planning staff to support the goal of permanency for children in foster care either through reunification with parents, adoption or preparation for independent living. The Case Planners are trained and supported in implementation of three evidence-based models used to support families and caregivers to assure a safe and permanent outcome for every child.
Essential Responsibilities:
- Creates, in partnership with families, plans which address safety, behavioral, psychological and permanency needs.
- Monitors safety, well-being, and permanency progress by conducting regular casework contacts and providing the necessary services to meet the needs of the children and families.
- Complete timely documentation of all reports and casework activities including FASPS, Psychosocial, Adoption Home studies, Kinship Homestudies, CANS assessments, court reports
- Participates actively in Family Court Hearings, Family Team Conferences, Administrative meetings and other meetings to advance permanency for children and families.
- Provide crisis intervention as needed
- Implement evidence-based models with foster parents, parents, discharge resources and youth.
- Coordinate with other agency and community-based programs such as Behavioral Health & Wellness and Health Homes
- Provide family planning counseling to youth 12 years and older and document discussion with foster parents
- Participate in all trainings including training for evidence-based models and other trainings in support of model implementation.
- Work to identify family resources and support for youth through Family Finding evidence-based practice.
Knowledge, Education and Skills Required:
- Bachelor's Degree in Social Work, Human Services, Psychology or related field required; Master's degree in Social Work or related field preferred
- Experience working with families and adolescents in the child welfare system required.
- Must be able to establish working relationships with participants in need of supportive services
- Ability to work with diverse population of clients and team members
- Must demonstrate excellent organizational skills with ability to meet deadlines
- Must have excellent verbal and written communication skills.
- Ability to work independently, as well as contribute to a team
- Ability to work flexible hours - some evenings as needed
- Ability to use public transportation to travel throughout New York City or Agency vehicle when available
- Must have a commitment to work from a strength-based, youth development and trauma informed perspective.
- Proficient using MS Office with the ability to learn new systems
- Bilingual English/Spanish preferred.
- NYS Drivers Licensed preferred.
Hourly Rate: $26.60 to $28.00 - depending on experience
*Candidates with Masters Degree in Social Work are subject for higher pay rate ($30/hr or more)*
