Job Description

RN Case Manager - Advanced Care - Educator

Responsibilities:

Research and organize/provide ongoing mandatory training opportunities, including, but not limited to BLS classes
Conduct facility audits and present results to facility management staff
Acute Educator: Minimum Experience: 5 years of full-time acute care dialysis experience as an RN
Assists Agency leadership with achieving corporate goals, analyzing data, improving patient care outcomes and patient satisfaction.
Participates in Agency quality assessment performance improvement initiatives.
Ensures that staff complete and maintain accurate, complete and timely documentation reflecting the assessment, intervention, education and evaluation according to Federal, Department of Health and The Joint Commission standards.
Assists in the recruitment, selection, development and retention of nursing staff.
