Job Description

Circle of Life Home Care is looking for a Client Coordinator to join our Metro, MN Team. Circle of Life is a leading home care agency operating in 6 states, with headquarters in Minneapolis, MN. We provide services for children, seniors, and people with disabilities, and our mission is to “honor those we serve”. We take pride in the responsibility we have caring for our clients so that they can stay independent in their homes and communities.

Salary Range: $18 - $22 per hour

Job Overview:

The Client Coordinator is responsible for managing and administering assigned programs, and will work with the Branch Manager to support general office operations. The Client Coordinator will ensure the operational process of healthcare services is efficient and meets clients needs

Essential Duties:

  • Determine if potential clients qualify for programs offered and secure necessary paperwork
  • Oversee the referral intake process for the office, including coordinating with outside partners to coordinate service, developing plans of care, conducting visits in the home, and ensuring quality service is provided to client
  • Onboard new clients to the company, including clients transferring to the agency
  • Resolve any concerns client or case manager may have with performance of services being provided
  • Market current services to potential clients, families, case workers, and others who could refer additional business to the organization
  • Review care plans for discrepancy or errors
  • Establish appropriate network and service linkages with Native Community
  • Attend community events to highlight the services offered by Circle of Life Home Care
  • Maintain an accurate client list, including monitoring the insurance status of all current clients
  • Input client data into electronic systems, ensuring information is entered timely and accurately
  • Attend weekly staff meetings
  • Assist with payroll process as needed
  • Performs other related duties as necessary or assigned

Required Qualifications and Experience

  • Technical school or one or more years of related experience and/or training; or equivalent combination of education and experience.
  • Knowledge of computer systems including Microsoft Suite with an emphasis on Outlook, SharePoint, Excel, and Microsoft Teams.
  • Ability to manage time, handle stressful situations, and work with deadlines.
  • Possess a willingness to assist, encourage, and honor those we serve.

Preferred Qualifications

  • Previous experience working in a home care environment.
  • Supervisory experience a plus.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:00 to 4:30. Possibility of working the occasional weekend for promotional events.
