Responsibilities include:

• Providing nursing services to participants in a specific program, specialty department, or practice.
• Conducting initial assessments and regular re-assessments, focusing on prevention, early detection, acute, and chronic illness.
• Collaborating with an interdisciplinary team to develop and implement individualized care plans.
• Performing technical skills according to hospital policies and accepted standards of practice.
• Evaluating care plans based on optimal patient outcomes and documenting participant/family response to care.

What you need to succeed:

• Nursing license in good standing.
• Strong computer skills, with Epic experience preferred.
• Willingness to work flexible hours within the office's operating schedule.
• Ability to adapt and collaborate effectively in a fast-paced healthcare environment.
