Position Overview

The Home Visitor for Family Foundations must be able to form relationships with children and families.  The Home Visitor provides opportunities for families to make informed decisions and create their own family goals and agendas.  This position is responsible for establishing partnerships with family members, other staff, and agencies to assess for strengths, skills, and needs.

The Home Visitor is responsible for the implementation of the Individualized Family Partnerships Agreement (IFPA) and for creating developmentally and age-appropriate activities to model for the family that will promote good outcomes for EHS enrolled Infants and Toddlers.  This person will carry out the agency and program philosophies to assist families in reaching their goals.Mission

Key Responsibilities
  • Visiting families in their homes and establishing working relationships
  • Collaborate with the site team to design and implement strategies that support compliance with the Early Head Start Performance Standards.
  • Promote optimal development of enrolled children across the five child development domains using, using the OUNCE curriculum
  • Provide appropriate parent education
  • Lead infant/toddler, parent and other group in socialization activities 2 times per month
  • Complete child development assessment every 4-6 months, and develop appropriate goals with family
  • Develop partnerships with families to create Individualized Family Partnership agreements
  • Plan and implement purposeful and meaningful home visits weekly and group socializations experiences (twice a month) based on individual child curriculum plan
  • Maintain a caseload of 10-12 families
  • Ensure appropriate assessments, brokering, advocacy, goal planning, case management, referral and support services to families
  • Facilitate access to other agencies and services
  • Enhance participants’ involvement in the broader community and in program governance
  • Recruit and enroll families for the EHS program
  • Maintain complete case records and implement Head Start Family Information System (PROMIS)
  • Attend all necessary program meetings
  • Coordinate bus drivers schedule (as necessary)
  • Maintain client confidentiality
  • Submit documentation in a timely fashion
  • Attend staff meetings and one-on-one supervision as scheduled
  • Attend agency and departmental training
  • Follow agency and program policies and protocols, including all personnel and regulatory requirements (e.g., TB test, bi-annual physical, etc.)

Knowledge Skills and Abilities

Through a combination of experience and education, the Food Pantry Specialist will have:
  • Experience with mobile and computer-based technology
  • Ability to communicate effectively both verbally and in writing
  • Must possess organizational skills, problem solving skills
  • Ability to function as a constructive and productive team member
  • Ability to establish and maintain boundaries
  • Ability to motivate and positively encourage clients to succeed
  • Ability to meet the physical demands of the job including lifting, bending, going up and down stairs.
  • Ability to work flexible hours including evenings and weekends
  • A valid driver’s license, a reliable vehicle, and comfort driving throughout the City of Pittsburgh and Allegheny County.
BS in Child Development or Early Childhood or degree in related field preferred with experience in family support, case management, infant/toddler development with 9 credits of childhood development classes or a minimum of 2 yrs. experience working in a family support program of EHS program with direct experience with infant and toddlers goal planning, home visiting and relationship building. Must be at least 21 years of age or older.
 
  • Must have Act 33 and 34 Clearances
  • Must have experience with residents in low-income neighborhoods and families with young children

Position Physical Demands:

The Home Visitor will be called upon to engage in physical activity related to general office work, including the capacity to lift to 25 lbs. and stand or sit for extended periods.
