Lakeview Village is searching for a caring, dedicated, and RN Case Manager to join our excellent Home Health team with a passion for healthcare.
Lakeview Village in Lenexa, Kansas is a 5-Star not-for-profit continuing care retirement community that offers sub-acute rehabilitation and long-term care, celebrating its 60th anniversary and 5th straight year as US News and World Report's Best Short-Stay Rehab.

Lakeview Village Home Health provides Medicare-certified, short-term, goal-oriented medical care and support in the comfort of the client’s own home. Home Health services are provided to residents of Lakeview Village, as well as to seniors living in Johnson, Wyandotte, and Douglas counties.

  • Provide comprehensive nursing services to individuals and their families in a community setting, with emphases on the promotion and maintenance of health, early detection, and restoration measures.
  • Provide leadership for Home Health Aides and Staff Nurses to ensure that care is properly and effectively implemented.
  • Thoroughly and completely record all symptoms, progress, and treatments in the appropriate charting system to ensure the continuity of care, maintenance of accurate legal documents, and caring, professional services.

Ideal candidates will have the following qualifications and skills:

  • Graduate of an accredited school of nursing
  • Possess a current, unrestricted RN state nursing license in the state of Kansas.
  • Current CPR certification
  • Case Manager certification (preferred)
  • Understanding and effective application of state, federal, and medical regulations as they apply to home health.
  • Prior home health and/or case management experience (preferred)
  • Reliable transportation and a valid state driver’s license
  • Excellent communication skills
  • Possesses and empathetic approach to the elderly.
  • Able to work in a team setting with residents, relatives, staff, and others.
