COMPENSATION: 
- $105,000 to $120,000+ Base Salary, negotiable dependent upon experience

COMPANY PROFILE: 
This physician-owned private practice operates an outpatient office in Port Charlotte and has a dedicated team of providers who visit local skilled nursing and assisted living facilities.  They provide a full scope of primary care services to adult and geriatric patients.  The practice is currently growing and is excited to welcome a new provider to their team.

POSITION DESCRIPTION: 
This is a full scope adult and geriatric primary care position in which the new provider will see patients in skilled nursing facilities (SNFs) and assisted living facilities (ALFs) in Venice.  

It’s an autonomous role in which the provider will have their own patient panel and clinical independence, with their supervising physician accessible when needed.

Duties include, but are not limited to:

- evaluation and management of adult / geriatric patients
- preventative medicine
- chronic disease management
- minor illness / injury care
- regular rounding

The new provider will have little-to-no travel during the day, visiting one (1) or two (2) facilities and seeing an average of 15 to 20 patients.

The rotation includes SNFs on Monday / Wednesday / Friday and a mix of SNFs and ALFs on Tuesday / Thursday, based upon patient need.

The practice uses the Elation Health EHR.

On-call is phone triage only and shared evenly with the other 4 APPs working in the SNF/ALF department.  Currently, on-call is 5 or 6 evenings per month and volume is low.

REQUIREMENTS:
- Florida NP or PA license
- DEA
- no experience required
