JOB SUMMARY

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.

RESPONSIBILITIES

• Provides outreach and engagement at community resources fairs and events to promote the programs and services.

• Provides in-person and/or virtual services in the community to bring cultural and linguistically appropriate health and mental health education and information to their communities through workshop facilitation and group discussions.

• Reduces stigma by engaging in dialogue with community members to normalize the experience of individuals living with mental health conditions and serves as role model to battle community biases.

• Advocates for the underserved, marginalized, and vulnerable populations within the mental health system of care and community at large.

• Assesses needs of individuals, families, and communities; utilizes training and shared lived experiences to recognize risk factors and protective factors.

• Attends community meetings to maintain, improve, and develop community partnerships.

• Documents activities including the service delivered, locations, type of settings, and basic demographic data of participants.

• Participates in on-going professional development, program trainings, peer-to-peer learning and meetings.

• Supports relations between the agency/department and the community by communicating agency policies and programs to clients, patients, parents of children/youth, family members, and community residents. Conveys and integrates feedback from clients.

• Serves as an advocate for client/patient access to departmental, program, and community resources.

• Assists clients, patients, family members, and caregivers in obtaining and completing application forms for benefits and services.

• Provides emergency services to clients by making referrals to appropriate supportive agencies and arranging for emergency shelter.

• Assists clients and patients in completing necessary forms to access services and benefits.

• Facilitates client development of independent living skills by conducting support groups in subjects such as housekeeping, cooking, shopping, household budgeting, use of public transportation, and attendance to personal grooming and hygiene. 1

• Facilitates client development of personal recovery/resiliency pathways by conducting individual coaching/mentoring sessions and/or by conducting support groups in subjects such as self-directed care, socialization activities, job seeking, and spiritual wellness.

• Promotes client community reintegration for greater access to services, resources, and supports.

• Comply with all TURN BHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS

• High School diploma or GED required.

• Bilingual in Spanish required.

• Promotores Community Health Worker training completed (CCHW) within 90 days.

• Mental Health First Aid training completed within 90 days.

• Knowledge of San Diego behavioral health system of care.

• Knowledge of SUD resources, mental health, and domestic violence resources within the community.

• Must be adept at problem-solving, including being able to identify issues and solve problems in a timely manner.

• Must be able to communicate clearly, both in writing and orally, with employees, management, and co- workers.

• Ability to create reports and spreadsheets.

• Highly organized, thorough, and able to self-monitor work for quality. Must be dependable, able to follow instructions, timely responding to management, and enjoy working in a fast-paced environment. Enjoy working in the community and as part of a team.

• Proficient user of Microsoft Office Suite and EHR software.

• Ability to problem solve and react appropriately to emergency situations.

• Ability to work independently and effectively collaborate with a team.
