Plymouth Psych Group, located in Plymouth, Minnesota, is a full-service mental health clinic serving patients of all ages and their families. We are currently seeking a Care Coordinator to join our team!

Schedule: Mon-Fri 9:00am-5:30pm
Job Summary:
The role of the Care Coordinator is to provide administrative support to individuals and/or families seeking care at Plymouth Psych Group, including greeting patients, verifying their information, scheduling appointments, answering phones, directing calls for patients appropriately, maintaining confidential medical records, collecting copays, co-insurance and deductibles. Sharing responsibility for requests, questions, and information communicated by patients. The Care Coordinator acts as the liaison between the patients, back office and clinical care staff.
Duties & Responsibilities:
- Greets all patients, verifies their personal information and insurance information is all recorded correctly.
- Assists patients or family members with completion of pre-registration forms.
- Look up accounts for no-call, no-shows to leave message to reschedule with scheduling.
- Coordinate last minute appointments and verify scheduling.
- Filing orders, patient demographics, requests and charts and shredding any outdated information.
- Informs patients of costs of care being provided.
- Collects and post payments according to protocols; prepare and balance daily financial registers and submit all forms and fees to the billing department.
- Answers telephone, screens callers, relays messages, and route to appropriate department.
- Making necessary appointments in Practice Management for all patients while keeping physician’s schedules coordinated.
- Responsible for checking voicemail every 30 minutes to ensure that all patient messages are attended to.
- Confirms call-backs, reschedules and no show/late cancellation policy
- Assist with filing and any additional support needs.
- Adheres to all applicable laws and regulations.
- Maintains confidentiality within the workplace and within the community.

- High degree of confidentiality, integrity and professionalism.
- Ability to handle protected health information in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Performs other related duties as required. This position is subject to change at the discretion of management and additional duties may be applied.
- Regularly shows up to work on time according to the established schedule.
Qualifications:
- Must have compassion and understanding for all patients and believe that patient care is paramount.
- Must have excellent customer service skills.
- Understands the importance of maintaining confidentiality according to HIPAA standards.
- Must be highly flexible and able to accommodate changing needs of the department.
- Must have basic computer skills, including Word and Excel.
- Must have the ability to multi-task, prioritize and organize.
- Must pay attention to detail while remaining focused in a busy environment.
