OVERVIEW

Responsible for providing lead case management and referrals to newly arrived refugees in their initial resettlement adjustment period including referrals to external services and other agencies, documenting services, filing and ongoing training to refugees. Assists the Program Director in providing leadership to the case management and support staff. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

ESSENTIAL FUNCTIONS
  • Provides program Case Management support and leadership to Case Managers
  • Provides ongoing trainings as needed
  • Oversees daily activities of the program in Director's absence
  • Coordinates weekly staffing meetings for high-acuity clients
  • Leads file reviews as part of Continuous Quality Improvement with Program Coordinator
  • Monitors and updates client financial records
  • Develops partnerships with other public and/or private organizations and employers that will result in employment
  • Develops, implements, and monitor self-sufficiency and employability plans for clients
  • Recruits program participants for the Vulnerable Care Services (VCS)
  • Maintains physical and web-based files in a logical, timely, and efficient manner
  • Provides weekly update report during department meetings
  • Provides emotional support and education to VCS Clients
  • Provides immediate case management services to clients to ensure that they develop a short-term and long-term plan for housing and employment
  • Completes the appropriate intake for clients for reception & placement (R & P), RSS, and other programs
  • Meets with clients to help with achievement of desired outcomes; completes monthly check-ins to ensure wellbeing and access to all basic needs
  • Responsible for ensuring clients successfully navigate the social services system in order to get the services requested; helps to remove any barriers
  • Documents all actions taken in databases and software
  • May Drive agency or personal vehicle on company business

MINIMUM REQUIREMENTS

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Executive Director of Human Resources.
  • Bachelor Degree in Human Services or related field
  • 3 years of experience in case management
  • Obtain CPR and First Aid trainings
  • Proven computer skills utilizing Microsoft Office Suite
  • Proven strong communication skills - oral and written are clear, concise, and in an organized fashion using appropriate grammar, style, and tone
  • Proven record of working effectively with families of varied lifestyles, backgrounds and ethnicities
  • Ability to lift 35 lbs. 10% of the time; ability to stoop, bend, climb and reach.

REGULATORY
  • Must be at least 18 years of age
  • Valid driver license, proof of insurance, and 39-month motor vehicle report
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid)
  • Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid)

DESIRED QUALIFICATIONS
  • Master Degree in human services or related field
  • Additional years of directly-related experience
  • Multilingual
