Description:

The Baldwin Women’s Residence (BWR) is a 50-bed transitional housing program for women and the Men’s Residence (MR) is a 28-bed transitional housing program for men. Both men and women are in recovery from addictions and utilize the 12 Step Recovery Program philosophy. This position is full-time, and reports to the Program Director. This position combines case management of residents along with increased duties to best assist the Program Director.


Supervisory Responsibilities

  • None


Program Coordinator Position Responsibilities:

  • Manage creating, altering, and deactivation of door access cards.
  • Maintain safety and security for the residence.
  • Creating and maintaining the client chores list and rotation.
  • Monthly report for women’s or men’s residence. Maintain monthly data for residential intakes and exits with accuracy. Maintain HMIS data for intakes with accuracy. Complete monthly data by the 3rd of each month.
  • Ability to demonstrate competent drug testing protocols and procedures, and the ability properly to administer urinalysis to clients.
  • Refer clients to other agencies for appropriate services as needed.
  • Provide regular status reports to the Program Director/manager.
  • Provide ongoing motivational and emotional support for the clients.
  • Liaison between the residents and other relevant professionals or programs as appropriate.
  • Complete all required documentation and case notes in an electronic client information management system in a timely manner.
  • Coordinate discharge planning process to ensure necessary support and services are in place. Provide support to the case managers.
  • Process intake paperwork for new clients efficiently and in accordance with SPFC protocols.
  • Create, change, and update intake forms and documents. Gather relevant medical history. Document information accurately and protect patient privacy according to HIPAA guidelines.
  • Greet visitors and future clients. Direct current clients to the appropriate department staff.
  • Other duties include coordinating appointment times for possible new clients, logging calls in electronic log database, assist callers with available resources, and send corresponding letters for certain inquiries.
  • Responsible for documenting and printing graduation certificates, movement recommendations to the Program Director or manager, updating bed roster for staff weekly meetings.
  • Maintain a compacity of no less than 85% in both BWR and MR.
  • Liaison between the residents and other relevant professionals or programs as appropriate.
  • All other duties as assigned.

Agency-wide Responsibilities

  • Maintains documentation and records in compliance with policies and procedures.
  • Maintains confidentiality.

Requirements:

Required Skills & Abilities

  • Strong relationship-building skills with both internal stakeholders.
  • Strong analytical and problem-solving skills.
  • Capability to prioritize inter- and cross-departmental collaboration.
  • Proficient in Microsoft Office Suite and related software.
  • Capability to use a broad range of technology.
  • Commitment to and modeling of St. Petersburg Free Clinic mission and values.
  • Must be able to pass a level 2 background check.

Education & Experience:

  • Associate degree required.
  • Knowledge of addictions and recovery preferred.
  • Minimum of one year case management experience.

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 25 pounds at times.
