Job Qualifications/Duties

Licensed in one of the following: Illinois, New Jersey or Wisconsin as an PMHNP or PA-C.
Prior experience in an outpatient mental health setting.
Conduct consultations for Medication Management as well as routine follow up visits
Perform patient evaluations and assessments as necessary
Develop treatment plans to support therapeutic interventions.
Maintain accurate and timely clinical documentation.
Collaborate with the treatment team and outside providers.
