Clinical Pharmacist Job Summary
The Clinical Pharmacist is responsible for providing pharmaceutical care to patients in a healthcare setting. The successful candidate will assess the medication needs of patients, review patients’ medication histories, and monitor the patient's progress and response to drug therapy. The Clinical Pharmacist will also consult with physicians and other healthcare professionals regarding optimal drug therapy, and provide information to patients on the safe and effective use of medications.
Clinical Pharmacist Duties and Responsibilities
- Review and interpret prescriptions, assess patient’s needs, and select and/or order appropriate medications and supplies
- Counsel patients on the proper use of medications, including dosage and potential side effects
- Provide drug information and education to patients, physicians, and other healthcare professionals
- Monitor and evaluate patient’s response to drug therapy, and adjust medications as needed
- Compound, package, label, and dispense medications according to established protocols
- Maintain records of medications dispensed and maintain patient profiles
- Stay informed on new drugs and pharmaceutical developments
Clinical Pharmacist Requirements and Qualifications
- Bachelor’s or doctorate degree in pharmacy
- State pharmacy license in good standing
- 3+ years of experience in a clinical pharmacy setting preferred
- Knowledge of relevant laws, regulations, and standards of practice
- Proficient in compounding and dispensing medications
- Excellent communication, organizational, and interpersonal skills
- Able to work independently and as part of a team
