Home Health Registered Nurse

THis is for a part-time/PRN position as a Home Health Visiting Registered Nurse. Area of coverage for the Chicagoland area and primarily in Cook, DuPage and Will counties. Our office is located in Woodridge, IL.  Coverage for patients will depend on a designated area for visiting that you choose. You get to define the area of coverage.

The Home Health Nurse candidate will provide nursing care in a patient's home. The candidate must be compassionate about caring for their patients, well-organized, accurately and efficiently caring for and documenting care according to the individual patients' needs. The candidate will admit, re certify, and discharge patients for home health care services, communicate with physicians, therapists, other clinicians, and the care coordination team. The candidate will also document visits, and oversee home health aides and as needed. The candidate must be proficient in wound care and other areas of nursing care. Understanding Medicare and insurance guidelines, rules, and requirements is required.

Requirements:
• Candidates must be a Registered Nurse with current Illinois license.

• Must be proficient and efficient to promptly document report / issue .

• Promptly and accurately submit patient OASIS and visit documentation

• Candidates must have a current CPR certification.

• At least one year of experience is preferred.

• Associates degree required as a minimum
