Job Description

Registered Nurse

Basic Functions: Act as the central medical member of a multi-disciplinary team of behavioral health professionals to assist the chronic and persistently mentally ill population, using a team approach, to keep them from repeat hospitalization, incarceration and to maintain recovery.

Complete Nursing Assessments, including both mental health and substance use disorders, assist consumers in maintaining medication compliance and monitor for physical symptoms or side effects of medications.

Qualifications:

  • Associates of Applied Science Degree in Nursing
  • Experience in Mental Health Setting
  • Licensed as Registered Nurse through the State of Michigan
  • Employee must have a valid Driver’s License with a driving record that is acceptable to the company’s insurance providers.
  • Employees must provide their own reliable transportation to and from the office workplace/work related locations and to transport consumers, as needed, (without delay) in a HIPPA compliant environment, with/without advanced notice.

Duties and Responsibilities:

  • Complete Nursing Assessments, including both mental health and substance use disorders, as scheduled; monitor for missing or outdated Nursing Assessments
  • Ensure that for all Medications, there is a current and complete Medication Consent
  • Administer Injectible Medications as prescribed by the Physician
  • Assist and educate consumers on any medical conditions that they may have
  • Monitor insurance status of consumer to assess their ability to obtain prescribed medications
  • Process all prescriptions in a timely manner
  • Attend work related meetings and trainings in a timely and professional manner
  • Process medication refills, under the direction of the Psychiatrist, as needed
  • Complete documentation related to all indirect and direct services within 24 hours of service delivery
  • Enhance professional growth and development through participation in educational programs, current literature, in-services, and workshops, including co-occurring mental health and substance use disorders
  • Complete Patient Assistant Program applications on behalf of consumers, as needed
  • Provides interdisciplinary health care services by use of high degree of collaboration and communication between all professionals about the needs of our members in relation to their comprehensive treatment plan (IPOS) addressing biological, psychological and social needs
  • Monitor status of lab orders, consumer follow through, lab reports, etc.
  • Ensure that Nursing supplies are fully stocked and that requests for supplies are made in a timely manner
  • Assist staff in the event of a psychiatric crisis or medical emergency
  • Participate in Person Centered Planning process of consumer/family; recommend Nurse related goals as appropriate and include co-occurring mental health and substance use disorders in the plan as necessary
  • Facilitate coordination between TMHS/consumer and community agencies, professionals, primary care physicians, etc
  • Maintain compliance with completion of Service Activity Logs and productivity expectations
  • Maintain compliance with Human Resource file, including training and education requirements (Note, 24 hours of additional training required for Children’s Program)
  • All other duties and responsibilities as assigned by the Program Director
