SH-758 ACT Registered Nurse - Petersburg, Virginia
Position Summary:
As an integral member of our multidisciplinary Assertive Community Treatment (ACT) team, the Registered Nurse delivers comprehensive psychiatric and medical nursing care. This role is responsible for serving as the primary clinician for a dedicated caseload, performing direct care including injections and phlebotomy, conducting nursing assessments, and managing client health needs. The RN coordinates closely with the team psychiatrist/PMHNP to administer medication, makes referrals to community providers, and provides a full spectrum of treatment, rehabilitation, and support services to adults with severe and persistent mental illness.
Essential Duties and Responsibilities:
Clinical Care & Treatment:
  • Serve as the primary clinician and provide psychiatric nursing services for an assigned ACT caseload.
  • Conduct ongoing assessments of clients' mental illness symptoms, medication effectiveness, and physical health needs.
  • Provide symptom education and teach behavioral symptom-management techniques to clients and their families.
  • Deliver direct clinical services on an individual, group, and family basis in both office and community settings.
  • Develop, revise, and implement client treatment plans in collaboration with the multidisciplinary team.
Medication & Medical Management:
  • Under the direction of the team psychiatrist/PMHNP, transcribe, administer, evaluate, and document all prescribed psychotropic medications.
  • Organize and manage the medication delivery system, ensuring integration into each client's individual treatment plan.
  • Coordinate, schedule, and administer medical assessments; make referrals to community physicians and coordinate follow-up care.
  • Perform nursing tasks including injections, phlebotomy, and routine monitoring of medication/treatment rooms.
  • Manage pharmaceuticals and medical supplies. Prepare medications for delivery by non-medical staff.
Coordination, Documentation & Compliance:
  • Coordinate the activities of the Individual Treatment Team (ITT), consulting with community agencies and families to maintain treatment cohesion.
  • Document all client progress, services, and communications accurately and promptly in the permanent record, maintaining a 95% compliance standard.
  • Ensure all documentation meets standards for Medicaid, Medicare, commercial insurance, and regulatory agencies.
  • Audit physician orders for medication changes, lab work, informed consents, and AIMS utilization.
  • Schedule nursing appointments (labs, injections) and assist in scheduling daily MD/PMHNP appointments.
Teamwork & Operational Support:
  • Participate in daily staff organizational meetings, treatment planning reviews, and shift management.
  • Provide side-by-side supportive services to assist clients with activities of daily living, including personal hygiene, meal planning, and grocery shopping.
  • Demonstrate excellent customer service, professional communication, and a commitment to teamwork.
  • Coordinate with inpatient services for client detoxification and linkage to outpatient programs and self-help groups.
General Responsibilities:
  • Prioritize tasks to meet productivity standards and deadlines with minimal direct supervision.
  • Attend all required meetings, staffings, and trainings to maintain current practice standards.
  • Work with payers to coordinate client eligibility, authorizations, and treatment.
  • Maintain strict patient confidentiality and conduct all services in compliance with laws, regulations, and agency policies.
  • Perform other related duties as assigned.
Qualifications:
Education & Experience:
  • Associate's degree in Nursing (ASN) required; Bachelor's degree (BSN) preferred.
  • Current, unrestricted Registered Nurse (RN) license in Virginia.
  • Minimum of one (1) year of clinical experience providing services to adults with severe and persistent mental illness; 1-3 years of experience in a similar community mental health setting preferred.
  • Current CPR/First Aid (FA) certification.
  • Valid Virginia driver's license with an acceptable driving record.
Required Knowledge, Skills, & Abilities:
  • A strong commitment to the principles of psychiatric rehabilitation and the right of individuals with mental illness to live in community-based settings.
  • Skills to establish supportive, trusting relationships with clients and respect for client rights and personal preferences.
  • Proven ability to work effectively as a member of a multidisciplinary team.
  • Proficiency with electronic health records and clinical documentation systems.
  • Flexibility to work evenings, weekends, and holidays as needed by the team.
  • Excellent clinical judgment, problem-solving skills, and the ability to work efficiently with minimal errors.
