About the Role

Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Registered Nurse to
deliver medical and psychiatric care to individuals with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system. As a Forensic ACT RN, you will provide medication management, health monitoring, crisis intervention, and care coordination while working closely with law enforcement, courts, probation/parole, and correctional healthcare providers to ensure continuity of care and successful community reintegration. This role emphasizes forensic treatment planning, risk assessment, and legal system collaboration to support justice-involved individuals.

Position Highlights:

  • Administer and monitor medications, provide health education, and ensure adherence to prescribed treatments.

  • Conduct health assessments and screenings to identify physical and psychiatric needs requiring medical or clinical intervention.

  • Collaborate with criminal justice and behavioral health partners to coordinate medical and psychiatric care for justice-involved individuals.

  • Support reentry and continuity of care for individuals transitioning from incarceration to community-based services.

  • Participate in multidisciplinary team meetings, providing medical input, crisis support, and documentation in accordance with regulatory standards.

Compensation

This position is a fixed rate, with compensation aligned to role responsibilities, experience, and contract parameters.

  • Compensation: $75,000

Responsibilities

  • Provide care and manage medications for people with mental health and substance use needs.
  • Monitor clients' health and respond to mental health or other urgent crises.
  • Coordinate care with law enforcement, courts, probation/parole, and correctional services.
  • Support clients returning to the community and connect them with ongoing services.
  • Education:
    • Graduation from an accredited nursing program.
  • Certification / Licensure Requirements:
    • Must be a Registered Nurse (RN) currently licensed to practice in the state of North Carolina.
    • Certifications: Crisis Prevention Intervention (CPI) certification required within 90 days of hire. CPR/First Aid certification required.
    • Valid Driver's License and Reliable Transportation required.
  • Experience / Knowledge Requirements:
    • Preferred: Experience in crisis intervention, emergency psychiatry, or correctional nursing.
    • Knowledge of criminal justice system processes, forensic risk assessment, and reentry planning.
    • Experience working with justice-involved individuals with mental illness and co-occurring disorders.
