Job Overview - Registered Nurse (ACT):
Step into a key role with our client as a Registered Nurse (ACT) in Bronx, NY. In this community-focused position, you will develop individualized health assessments, coordinate comprehensive care plans, and provide health education for program participants. Collaborating with the ACT team, you will ensure integrated physical and mental health services are effectively delivered to promote positive outcomes.
Compensation: $92,000/year
Location: Bronx, NY
Schedule: Monday to Friday, 9:00 AM - 5:00 PM

Responsibilities as the Registered Nurse (ACT):
  • Health Assessments and Planning: Conduct individualized health evaluations, develop nursing regimens, and monitor participant outcomes.
  • Team Collaboration: Work with the ACT team to integrate physical and mental health services into participant care plans.
  • Health Education: Facilitate group sessions on wellness topics such as HIV/AIDS, smoking cessation, and recovery strategies.
  • Care Coordination: Advocate for participants by arranging medical care, community support, and collaboration with psychiatrists and healthcare providers.
  • Medication Management: Administer IM injections and vaccines, oversee medication inventory, monitor side effects, and train ACT staff.
