Registered Nurse Home Health

HORIZONS HEALTHCARE SERVICES is looking for a Home Health RN in Northern Virginia. Registered Nurses will be responsible for the application of the nursing process and the delivery of patient care in compliance with all local, state, and federal regulations. Registered nurses provide a broad range of nursing services on a thorough assessment of the client's home health needs and development of an individualized care plan. All services are accredited by The Joint Commission.

Job Functions:

• Conduct patient assessments and evaluations to determine physical condition, medical histories, vitals, medications etc through OASIS-D documentation
• Advise family members and other supportive individuals about the level of care needed for the patient
• Assess signs and symptoms indicating physiologic and psychosocial changes
• Communicate with physicians, clinical managers other home health team member( PT/OT/ HHA) regarding patient care
• Collect, analyze, and interpret data and information from team members and documents
• Utilize med/surgical skills such as phlebotomy, infusion, wound care, and teaching for chronic diseases
• Medication administration and management
• Respond to inquires, concerns, and complaints from patients and their families
• Track and maintain medical supply inventory
• Ability and willing to drive to Patient's homes.

Qualifications:

• Must be in good standing with the Virginia Board of Nursing and have an active license
• Must be willing to travel in assigned territory
• Knowledge of principles, methods, and equipment used in nursing
• Must be computer literate and able to document electronically
• Excellent customer service skills
• 2+ years Nursing experience
