Job Description

DUTIES:

Direct support and training for individuals with developmental disabilities living in their homes; including all areas of daily life. Supervision and oversight of health, safety and welfare for 1-4 residents including observing physician’s orders, medical protocols, dietary guidelines and attending doctor appointments and follow ups. Daily meals preparation, nutrition monitoring as needed and provide home cleanliness and daily documentation and shopping. Supervision of self-administration of medication, provide lifting and transferring as required. Support and training in skills and goals as set by the individual support plans (ISP) and Positive Behavior Support Plans (BSP) of each resident. Transportation of residents to appointments, day programs and/or community activities. Training and care of residents with multiple disabilities, including all ADL(s). Must be able to lift 75 pounds.

SHIFTS: **OVERNIGHT STAY IS REQUIRED

MON-THU: 2pm-10pm/ 10pm-7am

FRI: 2pm-9pm/ 9pm-9am

SAT: 9am-9pm/ 9pm-9am

SUN: 9am-9pm/ 9pm-7am

PRN (AS NEEDED)
REQUIREMENTS:
Must be at least 21 years old
High School Diploma or GED
Valid Driver’s License
1-2 years of experience working with Adults with Developmental Disabilities.
Satisfactory Local Criminal Background Check and Motor Vehicle Report (MVR).
Current CPR and First Aid (FA) Certifications.
Current NEGATIVE (TB) Test or Clear Chest X-Ray.
Certified Nurse Assistant (C.N.A) preferred.
