At Home Healthcare - Metairie, LA Seeking professional and caring RN’s to join the premier Home Health agency in the Greater New Orleans area. We value nurses who are motivated, resourceful, and passionate about providing excellent patient care to homebound patients. We offer a user friendly EMR system, efficient processes, comprehensive training, and a supportive management team. RESPONSIBLE AND ACCOUNTABLE FOR: • Coordinating and managing the clinical care of patients within a reasonable geographic area. • Conducting evaluations and patient assessments to determine CMS criteria for home health services. • Collaborating with clinical nurse managers, therapists, social workers and other team members to develop patient specific plans of care. • Providing ongoing evaluations and make necessary revisions as the patient’s needs change. • Performing comprehensive patient assessments. • Providing routine follow up visits. • Performing clinical tasks and administering treatments to patients. • Educating patients and their families on treatment protocols, health strategies and procedures. • Identifying discharge planning needs as part of the plan of care. Initiate referrals to other disciplines, community resources and alternate medical services as indicated. REQUIREMENTS: • Current license to practice as a Registered Nurse in Louisiana • Minimum 1 year of acute nursing experience • Current BCLS • Valid driver’s license and reliable transportation PREFERRED REQUIREMENTS: • Home health nursing experience • Strong clinical skills • Critical thinking and problem-solving ability • Proficiency with Home Health clinical documentation • Use of the OASIS tool and basic computer skills
