Responsible for discharge planning
What You Need to Bring to the Table:
MSW (Masters Degree in Social Work)
Must have Discharge Planning and Hospital Case Management experience

Sunday - Monday will be working 5 days a week 8 hours shifts, MUST be available on the weekends.
Start: ASAP they cleared. They need MMR, Varicella, PPD, Fit Mask Questionaire, Physical, Drug and Background. MSW degree and CPR certified
