Job Description

Summit Health is seeking a Social Work Care Manager with experience to join a care management team focused on assisting patients with identifying the next best site of care, reducing inpatient stays, and facilitating safe transitions of care. This Social Work Care Manager will partner with patients, families, and the care team to improve patient experience and outcomes for SMG patients admitted to acute care and subacute facilities, high-risk patients identified by payers. Responsibilities may include development of care plans, participation in utilization review rounds, collaboration with external agencies, case conferences, and other appropriate social work interventions aimed at efficient utilization of health care services.

Essential Job functions:
  • Works with multidisciplinary transitional care team to facilitate seamless patient transitions across the continuum of care.
  • Assesses assigned population of patients. Independently prioritizes care management needs (high need, high-risk and/or high cost).
  • Applies care management concepts, principles, and strategies to assist the high-risk patient in achievement of prevention and treatment goals.
  • Care plan development and monitoring for the highest need/highest cost patients in collaboration with the providers.
  • Provides interventions and coordination of social services to identified patients (pediatric - geriatric) with complex health and social needs, in collaboration with physicians and other care team members
  • Documents appropriately in the electronic health record, care management registry and any required patient tracking documents. Employs appropriate and timely use of tasking in the EHR. Reviews and updates medication and problem lists.
  • Performs all care management activities across the continuum of care while adhering to the core values of patient confidentiality, privacy, safety, advocacy, and adhering to ethical, legal, and accreditation/regulatory standards.
  • Delivers care management services within the scope of licensure in accordance with SMG policy.
  • Assumes accountability for the quality of care.
  • Continually seeks new knowledge and learning regarding comprehensive primary care and chronic disease management.
  • Other duties as assigned.
  • Strives to promote patient wellness, improved quality outcomes, efficient utilization of health services, and appropriate social work interventions for patients and caregivers.

Payer Contracts:
  • Demonstrates a broad understanding of payer contracts (e.g., understanding patient attribution, care management requirements, clinical quality metrics, and utilization and cost outcomes). Ability to work with payer provided reports.

Competency, Training and Education:
  • Maintains core proficiencies /competencies as defined by the Care Management Program.
  • Continually seeks new knowledge and learning regarding comprehensive primary care and chronic disease management.
  • Time and task management.
  • Self-motivation and focus.
  • Ability to work independently.

Quality Improvement Initiatives:
  • Serves as a resource for all current and future quality improvement initiatives within SMG.
  • Demonstrates ability to appropriately document data discretely in the EMR and serves as a resource for clinical office staff.

Physical Job Requirements:
  • Physical mobility, which includes movement from place to place on the job, taking distance and speed into account.
  • Physical agility, which includes ability to maneuver body while in place.
  • Dexterity of hands and fingers.
  • Endurance (e.g. continuous typing, prolonged standing/bending, walking).


Education, Certification, Computer and Training Requirements:
  • Education: Master of Social Work, Psychology or Related Discipline.
  • Experience: Three years of Care Management experience (such as Global Options or MLTSS), behavioral health background (mental health, substance disorders, crisis management), pediatric social work background
  • Comfort in learning and using digital tools.
  • Proficient Microsoft Office 365 skills required.
  • Ability to work remotely.
  • Able to work well in a virtual environment.
  • Meet technical requirements to work remotely.
  • Experience with standard office equipment (phone, fax, copy machine, scanner, email/voice mail) required.
  • LCSW preferred. Case Management Certification (CCM) preferred
