RN Case Manager | Apple Valley, CA
Location: Apple Valley, California
Assignment Length: 13 Weeks
Schedule: Day Shift | 5x8 Hours | 8:00 AM – 4:30 PM
Job Summary
"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager for an acute care hospital assignment in Apple Valley, CA. The ideal candidate must have strong hospital-based case management and discharge planning experience with the ability to work independently in a fast-paced healthcare environment.
Required Qualifications
• Active California RN License required
• Minimum 2 years of Acute Care Case Management / Discharge Planning experience required
• Hospital experience required
• First-time travelers will not be considered
• Strong communication and coordination skills
• Ability to manage high patient caseloads effectively
Key Responsibilities
• Coordinate patient discharge planning and continuity of care
• Collaborate with physicians, nursing staff, and interdisciplinary teams
• Review patient care plans and utilization needs
• Arrange post-discharge services and resources
• Maintain accurate patient documentation and case records
• Support patients and families throughout hospitalization and discharge process
• Ensure compliance with hospital policies and care management standards
