Per Diem Hospice Registered Nurse (RN) Intake/Admissions - Community Hospice

Join The Community Hospice as part of our Central Intake & Admissions Team, where your expertise helps ensure patients and families receive compassionate, seamless care when they need it most.

Why This Role Matters

As a Hospice Intake & Admissions RN, you are often the first point of contact on a patient's hospice journey. Your work sets the tone—providing comfort, clarity, and reassurance while guiding patients and families through the transition to hospice care.

What You'll Do

  • Serve as a key liaison between patients, families, providers, and our interdisciplinary team
  • Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams
  • Meet patients where they are—at home or in care facilities—to complete initial assessments
  • Initiate care plans, including ordering medications, medical equipment, and supportive services
  • Partner with social workers, chaplains, aides, therapists, and volunteers to provide holistic care
  • Ensure a smooth, supportive handoff to the Hospice Case Manager

Where You'll Work

This role offers a dynamic schedule, combining:

  • Time in our central intake office in Albany
  • Field visits to patients in homes, nursing homes, and assisted living communities across the region

What You Bring

  • Active New York State RN license
  • ASN required; BSN preferred
  • Home care experience (1+ year) strongly preferred
  • Hospice or end-of-life care experience is a plus
  • Valid driver's license and reliable transportation
  • Strong communication skills—you connect easily with patients, families, and clinical teams
  • Clear, concise documentation abilities
  • Comfort with computers and electronic health records
  • Excellent organization and time management skills
