Job Description Registered Nurse (RN) - Hospice Case Manager Location: Houston, TX Overview: Our hospice team is seeking our Hospice Case Manager to provide compassionate, patient-centered end-of-life care in home and community settings. Our field-based role offers autonomy, interdisciplinary collaboration, and the opportunity to make a meaningful impact while managing your own caseload. Position Offers: Schedule: Monday-Friday, 8:00 AM-5:00 PM Competitive salary: $95,000-$105,000 annually, depending on experience Health insurance 21 days Paid Time Off, Sick Time Off (STO), and paid holidays Mileage reimbursement Permanent, field-based position Collaborative interdisciplinary team environment Key Responsibilities: Manage and coordinate hospice patient care Perform patient assessments and develop individualized care plans Provide symptom management and patient education Collaborate with the interdisciplinary team and healthcare providers Coordinate patient services and support continuity of care Maintain accurate documentation and regulatory compliance Advocate for patients and families throughout the hospice journey Qualifications: Active Texas Registered Nurse (RN) license ADN required; BSN preferred Current BLS certification Minimum of 5 years of hospice nursing experience Strong case management, documentation, and communication skills Valid driver's license and reliable transportation Why Join Us: Our Hospice Case Manager RN role offers the opportunity to provide meaningful hospice care while enjoying a consistent weekday schedule, competitive compensation, comprehensive benefits, and the support of a collaborative team.
