Basic Function:

The Case manager position is an integral part of the client's experience at Lighthouse Central Florida. The primary duties include providing case management services for vision rehabilitation clients, tracking and management of caseloads and pre/post assessments, collaborating with other departments as needed, and managing the overall plan of care for each client.
Essential Job Requirements:
  • Provide counseling, guidance, or intervention to clients and their families to maximize the benefits of participation in vision rehabilitation.
  • Collaborate with clients to gather necessary information and documentation to ensure eligibility for appropriate provision of vision rehabilitation services.
  • Work cooperatively with the Division of Blind Services, Veterans Administration, eye care physicians, and other referring partners to facilitate delivery of vision rehabilitation services.
  • Facilitate appropriate outcomes for clients through the use of case management tools and resources by using critical thinking and judgement throughout the vision rehabilitation process.
  • Collaborate with internal and external parties throughout the service delivery process to ensure the best possible outcomes for clients.
  • Coordinate and implement assigned care plan activities and monitor plan progress.
  • Identify and escalate quality of care issues through established channels.
  • Utilize influencing/motivational interviewing skills to ensure maximum client engagement and promote lifestyle/behavior changes to achieve desired outcomes.
  • Ensure clients have the tools to actively participate in their vision rehabilitation outcomes.
  • Utilize case management and quality management processes in compliance with regulatory and accredidation guidelines and company policies and procedures.
  • Evaluate client satisfaction and quality of services provided.
  • Collaborate with the Client Experience Administration Team to ensure clear communication between client and vision rehabilitation instructors.
  • Serve as LCF representative for required meetings with State, local and other government agencies and other external organizations where case management and assessment functions are requested.
Other Job Requirements:
  • Comply with agency policies and procedures, including the completion of required documentation and reports.
  • Participate in agency activities and events as requested
  • Maintain confidentiality of program participants in accordance with State and Federal regulations.
  • Participate in and support the values and mission of Lighthouse Central Florida.
    • Perform other duties as required.
Qualifications

Qualifications
Education:
  • Bachelor's Degree in Social Work, Human Services, or related field.
License and/or Certification:
  • Certification in Case Management, Psychology, or Social Services preferred.
Experience:
  • At least 2 years working with individuals with disabilities is preferred.
  • At least 2 years of case management experience is preferred.
Demonstrated Knowledge and Skills:
  • Possess a strong business sense and can efficiently prioritize and make sound judgment calls when needed.

  • Ability to build strong rapport with clients and families quickly

  • Sound professional judgement and maturity.
  • Excellent time-management and problem-solving skills.
  • Excellent communication skills verbal and written English.
  • Bilingual (English/Spanish) strongly preferred.
  • Ability to understand and interpret pertinent policies and procedures clearly and accurately.
  • Knowledgeable in the use of technology, including databases and reporting suites.
  • Proficient in Windows, including the Microsoft Office business application suite; and data management systems.
  • Ability to perform detailed work with numerical data and to make arithmetic computations accurately.
  • Capable of working with a variety of personalities and leadership styles.
  • Detail-oriented and highly organized; capable of effectively balancing multiple priorities.
  • Flexible with workload and schedule to meet demands of the job.
Physical Demands:
  • Ability to remain in a normal seated position regularly.
  • Ability to move and lift items usually ranging up to 25 lbs.
  • Frequently listening and talking with clients, staff, visitors, or others.
  • Occasional travel is required.
Requirements:
  • Must posess reliable transportation.

Must pass Level II background check.
