We are currently seeking experienced Family Nurse Practitioners to join our growing team in Chicago, IL, and surrounding locations, with our headquarters in Oak Park.

Job Description: We are looking for experienced Family Nurse Practitioners (FNPs) to provide sub-acute care in post-acute care network Skilled Nursing Facilities (SNFs) and collaborate with our multidisciplinary team to deliver high-quality care. This role requires a commitment of 40 hours per week, with potential on-call duties during some weekends. The FNP will also be involved in quality improvement initiatives and teaching services within client SNFs, as well as assist in the development of High Acuity Units.

Key Responsibilities:

  • Commit to 35 to 40 hours per week, rounding at assigned facilities (SNFs, hospitals, clinics).
  • Potential on-call duties during some weekends.
  • Provide sub-acute care to patients at post-acute care network SNFs.
  • Organize and deliver quality improvement and teaching services to nursing staff within client SNFs.
  • Assist with the development of High Acuity Unit(s) within the SNF setting.
  • Collaborate with a multi-disciplinary team of primary care physicians and other allied clinicians to provide compassionate and high-quality care within acute and post-acute settings.
  • Develop patient care plans and work closely with nursing staff and patients' families.

Required Position Qualifications:

  • Current, unrestricted Family Nurse Practitioner (FNP) License in Illinois.
  • NP national certification.
  • DEA License and IL Control Substance License.
  • Clinical background in adult (ANP), geriatric (AGNP), or family specialties (AFNP) (our practice age range covers adolescence to older adults).
  • Proficiency in typing and computer skills.
  • Preferred past experience with SNFs and a solid understanding of post-acute care services.
  • Interest in senior services.
