ENROLLMENT SPECIALIST - HYBRID
JOB SUMMARY
Under the direction of the Director of Business Development, the Enrollment Specialist is responsible for guiding prospective participants and their families through the enrollment process and converting qualified referrals into program enrollments. This role serves as the primary point of contact for prospects, building trusted relationships while helping individuals understand the benefits and requirements of the PACE program.
Enrollment Specialists represent BoldAge PACE with professionalism and purpose, embodying our values of People First, Seek to Understand, Exceed Expectations, Do the Right Thing, and Be Bold. They work closely with Outreach Specialist, Enrollment Advisors, Benefits Specialists, Level of Care Assessors, and the Interdisciplinary Team to ensure a seamless and supportive experience for each prospective participant from referral through enrollment.
This role combines strong interpersonal skills, consultative enrollment conversations, and coordination across teams to help older adults and their families make informed decisions about joining the BoldAge PACE program.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
  • Assume ownership of prospects upon handoff from the Enrollment Advisor when prospects are prepared to move forward with enrollment or require in-person engagement to support decision-making.
  • Guide prospective qualified participants and their families through the enrollment process from post-intake through completed program enrollment, conducting consultative conversations to understand needs and align them with PACE services and benefits.
  • Clearly present program services, eligibility requirements, and enrollment expectations to prospective participants, caregivers, and family members.
  • Convert qualified referrals into enrollments by leading prospect decision-making and leveraging in-person experiences, including tours and Day-in-the-Life visits, to build trust and drive enrollment.
  • Coordinate and execute all enrollment activities with internal partners, including Benefits Specialists, Level of Care Assessors, and the Interdisciplinary Team, to ensure enrollment readiness and completion.
  • Actively manage enrollment timelines to ensure timely progression from qualification to enrollment, identifying and resolving delays or barriers.
  • Travel within the service area to meet prospects in their homes, healthcare facilities, or community settings to support enrollment.
  • Complete and finalize collection of all required enrollment documentation, including obtaining medical records and coordinating with healthcare providers and partners, using in-person engagement when necessary to ensure all forms, consents, and records are secured to support enrollment.
  • Conduct the Enrollment Conference and finalize participant enrollment in accordance with CMS regulations, including execution of the Enrollment Agreement and Interim Care Plan and provision of all required enrollment materials.
  • Support prospective participants and families in navigating the Medicaid eligibility process in collaboration with the Benefits Specialists and state agencies.
  • Represent the needs and goals of prospective participants during discussions with the interdisciplinary team.
  • Maintain responsive communication with prospects, caregivers, and referral partners throughout the enrollment process.
  • Maintain timely and accurate documentation of prospect interactions and enrollment activities in the CRM and EMR systems.
  • Support new participants and caregivers during the initial transition into the PACE program, including the first three months following enrollment.
  • Receive and execute warm handoffs from Enrollment Advisors, partnering with Outreach Specialist and internal teams to ensure a seamless transition from referral generation through enrollment.
  • Participate in community outreach activities when needed to support enrollment opportunities.
  • Participate in team meetings, continuing education, and professional development to maintain knowledge of PACE principles and messaging, policies, and compliance standards.
  • Maintain confidentiality, uphold all OSHA and infection control protocols, and contribute to a safe and positive work environment.
  • Support organizational quality improvement initiatives and represent BoldAge PACE with integrity in all internal and external interactions.
  • Perform other duties assigned to support organizational goals and BoldAge PACE operations.
EXPERIENCE AND EDUCATION
  • High school diploma or equivalent required
  • Bachelor's degree in Marketing, Business, Healthcare Administration, or Human Services preferred.
  • Minimum 1-3 years of experience in a customer-facing role required; experience in healthcare intake, call center operations, patient access, admissions, or inside sales preferred.
  • Experience managing high-volume inbound and outbound communications, including phone, text, and email.
  • Demonstrated ability to prioritize tasks and manage multiple prospects in a fast-paced, performance-driven environment.
  • Experience working with CRM systems and maintaining accurate, timely documentation
  • Knowledge of PACE, Medicaid, Medicare, or health care programs preferred
  • Proficiency in Microsoft Office suite.
  • Strong written, verbal, and interpersonal communication skills.
  • Experience working with older adults, caregivers, or vulnerable populations preferred.
PRE-EMPLOYMENT REQUIREMENTS:
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
