Job description

Job Description
This P/T position at our 72nd Street Clinic provides initial contact for our prenatal patients and maintains case records, assists with appropriate documentation relevant to patient's accounts, assess psycho-social status/risk factors of prenatal and postpartum clients and collaborates with providers regarding patient status.
This position also assists the Medicaid Office to screen eligible patients, organize/collect information needed for interviews, performs recalls as needed to maintain continuous enrollment in state health programs, collaborates with outside agencies to increase access and assist patients with assessing community resources such as WIC, Department of Human Services, Department of Housing, Workforce Services, and other state programs. Develops cultural awareness and utilizes second-language materials in conjunction with the Patient Support Services Director.
Knowledge of various State programs/services such as Medicaid, CHIP, Medicare and Baby-Your-Baby is a must. Bilingual English/Spanish (effectively communicate both verbally and in writing) is required to apply for this position.
Experience and Skills
Must be an organized self-starter, work independently of supervision, and effectively communicate both verbally and in writing. Previous experience working with cultural sensitivity and pregnant women is preferred.
