Location: Suffolk County, Long Island | Full-Time/Every Other Weekend | Non-Profit Healthcare

At Visiting Nurse Service and Hospice of Suffolk (VNSHS), our clinical teams deliver compassionate, high-quality care to home care and hospice patients in the comfort of their homes. As a Licensed Practical Nurse (LPN), you’ll provide skilled nursing care under the direction of a Registered Nurse while supporting patient health, independence, and quality of life.

Your Impact:

The Licensed Practical Nurse provides skilled nursing services under the direction of a Registered Nurse to a diverse population of home care and hospice patients, supporting patient care, monitoring health status, and ensuring timely communication and documentation.

You Will:

Provide skilled nursing care to home care and hospice patients under RN supervision

Adhere to infection control standards and agency policies and procedures

Utilize chronic disease management, health promotion, and motivational interviewing techniques

Administer medications and perform direct patient care

Observe, measure, record, and report patient condition and changes

Utilize telehealth as appropriateCare Coordination & Communication

Participate in care management and interdisciplinary team communication

Attend weekly meetings and collaborate with care team members

Communicate effectively using phone, email, and documentation systems

Documentation & Productivity

Complete accurate and timely documentation using point-of-care technology

Ensure visit notes are completed and submitted within 24 hours

Support insurance authorization processes as directed

Maintain productivity expectations (average of 6 visits per 7-hour day)

Professional Standards

Support the mission and values of VNSHS

Maintain professionalism and follow uniform policy

Participate in required trainings, competencies, and continuing education

Full time schedule including every other weekend

Assist with holiday coverage as needed

What You Bring:

New York State LPN license with current registration (required)

Minimum 2 years of LPN experience (home care or hospice experience preferred)

Ability to work under the direction of an RN

Strong communication, organizational, and clinical skills

Ability to work independently and as part of a team
