Advanced Practice Provider (APP)ย - Full-Time / Part-Time / Per Diemย 

Job Descriptionย 

The Advanced Practice Provider (APP), which includes Nurse Practitioners and Physician Assistants, is an essential member of the Local MD Urgent Care team. The APPย is responsible forย setting and upholdingย high standardsย of care quality and service excellence. Through consultation and collaboration with theย Medical Director, the APP provides comprehensive evaluation, diagnosis, treatment, and management of patient health needs.ย 

The APPย utilizesย independent clinical judgment, professional communication, and collaborative teamwork to deliver highquality care across the continuum. The role encompasses clinical leadership, patient education, partnership with staff, and ongoing contributions to quality improvement and clinical research. APPs build andย maintainย effective relationships with patients, families, physicians, and staff to achieveย optimalย outcomes.ย 

Essential Job Functions & Competenciesย 

Patient Screening & Analysisย 

- Performs initial patient assessments, including history-taking and physical examinations.ย 

-ย Establishesย baseline health profiles and analyzes diagnostic data.ย 

-ย Determinesย need for follow-up appointments,ย additionalย testing, or changes in treatment plans.ย 

Diagnosis & Treatmentย 

- Diagnoses andย treatsย acute and chronic health conditions including illnesses, infections, and injuries.ย 

- Prescribes medications based on efficacy, safety, cost considerations, and legal authorization.ย 

- Manages patient care plans and coordinates ongoing treatment needs.ย 

Professional Care Service Recommendationsย 

-ย Recommendsย appropriate on-siteย and off-site medical services within scope of training and specialty.ย 

- Coordinates referrals and collaborates with specialty providers as needed.ย 

Patient Educationย 

- Educates patients, family members, and guardians on self-management of acute and chronic illnesses.ย 

- Tailors education and instructions to each patientโ€™s unique circumstances and level of understanding.ย 

Patient Recordsย 

- Preparesย accurate, thorough, andย timelyย documentation in the EMR per Local MD standards.ย 

- Uses the EMR to complete documentation, medication orders, diagnostic orders, and referrals.ย 

Education & Trainingย 

- Provides medical direction, education, and training to clinical team members involved in patient care.ย 

-ย Demonstratesย leadership through mentoring, communication, and modeling best practices.ย
