Job Description

    Compensation Notes:ย 

    • $32 - $35 [$35 only for RNs with 5+ years' experience and BSN or MSN!]
    • Shift Differentials for Health Coaches:
      • Hours worked after 5pm = $4 / hour
      • Hours worked on Saturday and Sunday = $2 / hour

    Shift:

    • Monday - Friday 11:30 a.m. - 8 p.m. (Shifts will be later when candidates are working from home; Example: They will work 12:30 p.m. - 9 p.m. or 1:30p.m. - 10 p.m.)
    • Rotating weekends, every 3rdย Saturday and Sunday with days off during the week.
  • These RN Health Coaches will be talking to members about their healthcare goals.
  • They'll conduct telephonic assessments, create care plans, and guide callers through their healthcare choices.
  • They'll be handling both inbound and outbound calls to engage members in a coaching relationship.
  • Provide ongoing disease management coaching and symptom support education to members.

Company Job Description/Day to Day Duties:

  • The Health Coach provides unbiased evidence-based health information to assist individuals telephonicallyย with any health related concern.
  • The primary focus is to educate and guide a caller through their healthcare choices in order to be a more active and informed participant in the healthcare process.
  • That includesย general symptom management and answers to urgent questions, assistance and support in managing chronic or serious medical conditions, help preparing for important discussions with an individual's doctor, and general health related information and management.
  • Educate and guide a caller through their health care choices by accessing approved health coaching toolsย and websites, library resources, Shared Decision-Makingย materials for research. Provide education andย resources to client members.
  • Document calls, issues and general information in the company's computer database for trackingย member/patient information.
  • Perform assessments as indicated.
  • Assist callers in developing action plans to manage specific conditions.
  • Assist in the development of department annual goals and identification of outcomes for continuous qualityย improvement.
  • Seek opportunities to increase knowledge and expertise in a specialty area. This includes theย dissemination of information to the Health Coach team.
  • Function as a resource and mentor to new employees, as well as assist in assessing the ongoingย educational needs of the department.
  • Participate in case conferencing as needed in conjunction with peers and the leadership team
Qualifications
  • MUST haveย TX nursing license with multi-state privileges and be able to be licensed in all states (CA takes the longest)
  • MUST be bilingual - verbal AND writtenย in English and Spanish - Prior to submission, please have candidate complete the Spanish-English Bilingual Prove-It Assessment and include their score in the blurb. Must be above an 80%!
  • Minimum 3+ working as aย RN in a clinical setting
  • Strong communication skills, engaging to speak with over the phone
  • Strong computer skills able to easily navigate MS Word, Excel, Outlook; good keyboard skills
