About the Job:
  • Patient Care Coordinator - Health Coach.
  • We are seeking a Patient Care Coordinator - LVN/LPN or RN.
  • Must have a current nursing license.
  • You will coordinate a care and coaching program with remotely monitored patients.
  • This program focuses on diet and exercise. An increased knowledge based of macronutrients, calories, strength training, cardio, etc. is preferred.
  • This is a 100% work from home position. Anywhere in the USA.
  • Some latitude to create your own schedule.

Responsibilities:
  • Communicate care information from programs.
  • Engage new patients enrolled.
  • Help patients track and report changes in condition.
