Spanish Speaking MSW - Hospice/Home Health
Salus Hospice - Orange County is seeking a compassionate and dedicated Hospice MSW to join our team in Irvine, CA. In this role, you will provide essential social work services to patients and their families, ensuring emotional support, counseling, and assistance in navigating end-of-life care. You will be a vital part of our interdisciplinary team, working to enhance the quality of life for our hospice patients.
Responsibilities
  • Provide counseling and support to patients and their families to address emotional, social, and spiritual challenges.
  • Coordinate with interdisciplinary team members to ensure comprehensive care delivery.
  • Assist patients and families in accessing community resources and benefits.
  • Document all social work activities and maintain accurate patient records in compliance with company policies and regulatory requirements.
  • Participate in team meetings and case conferences to discuss patient progress and challenges.
  • Advocate for patients' rights and preferences throughout their hospice care journey.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in California.
  • Must speak Spanish
  • Experience in hospice, palliative care, or related health care settings preferred.
