Looking to hire a Full-time Social Worker Assistant for our skilled nursing facility. Join us in making a difference in the lives of those we serve by providing exceptional care and support.
Educational Requirements (preferred):
An individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and one year of supervised social work experience in a health care setting working directly with individuals.
Experience Required: Minimum of 2 years' experience in the social work field as a social worker; Minimum of 1-year supervised experience in a health care facility in the Social Services department
Essential Functions:
Maintain confidentiality and privacy of all patients
Ensure that the medical, social, and emotional needs of each resident are provided for
Provide crisis intervention, counseling, or other services to residents and families
Assist new residents and families in the initial adjustment phase
Work with residents and families on planning discharges
Coordinate social services to ensure the daily social services can be adequately provided
To provide a full range of medical, social and case management services to meet the bio-psycho-social, financial, emotional and cultural needs of patients, outpatients and their families
Identifying individual social and emotional needs
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Utilizes high risk criteria and/or consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention
Formulates and implements treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice
Performs related duties as required
Coordinating admissions and discharges to ensure facility census is kept at its maximum capacity
Maintain written documentation in the medical record per facility policy and state and federal guidelines
Communicates and interprets policies and procedures to all applicable staff, and monitors staff practices and implementation
Participate in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns
Demonstrates knowledge of and application of Key Clinical Quality Indicators.
Ensures that assigned resident assessments and care plans are incompliance
Actively participates as a member of the QAA Committee
