Looking to hire a Full-time Social Worker Assistant for our skilled nursing facility. Join us in making a difference in the lives of those we serve by providing exceptional care and support.

Educational Requirements (preferred):

An individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and one year of supervised social work experience in a health care setting working directly with individuals.

 

Experience Required: Minimum of 2 years' experience in the social work field as a social worker; Minimum of 1-year supervised experience in a health care facility in the Social Services department

Essential Functions: 

  • Maintain confidentiality and privacy of all patients

  • Ensure that the medical, social, and emotional needs of each resident are provided for

  • Provide crisis intervention, counseling, or other services to residents and families

  • Assist new residents and families in the initial adjustment phase

  • Work with residents and families on planning discharges

  • Coordinate social services to ensure the daily social services can be adequately provided

  • To provide a full range of medical, social and case management services to meet the bio-psycho-social, financial, emotional and cultural needs of patients, outpatients and their families

  • Identifying individual social and emotional needs

  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

  • Utilizes high risk criteria and/or consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention

  • Formulates and implements treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice

  • Performs related duties as required

  • Coordinating admissions and discharges to ensure facility census is kept at its maximum capacity

  • Maintain written documentation in the medical record per facility policy and state and federal guidelines

  • Communicates and interprets policies and procedures to all applicable staff, and monitors staff practices and implementation

  • Participate in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns

  • Demonstrates knowledge of and application of Key Clinical Quality Indicators.

  • Ensures that assigned resident assessments and care plans are incompliance

  • Actively participates as a member of the QAA Committee
