Position Overview

Waterview Heights is seeking a compassionate and skilled Social Worker to join our team. In this role, you will provide essential psychosocial support to residents and their families, ensuring the highest quality of life for our residents through personalized care and counseling.

Key Responsibilities

Resident & Family Support

  • Conduct interviews with residents and their families to assess psychosocial needs.

  • Provide individualized psychosocial assessments, counseling, and guidance to residents and their families.

  • Offer grief and loss counseling to residents and families experiencing difficult transitions.

Care Plan Development

  • Develop personalized care plans tailored to the individual needs of residents, in collaboration with the healthcare team.

  • Assist residents in navigating and applying for appropriate social services and community resources.

Crisis Intervention & Support

  • Provide crisis intervention services as needed to address immediate concerns or challenges.

  • Offer supportive counseling for emotional and psychological well-being.

Communication & Collaboration

  • Strengthen communication between residents, families, and facility staff to ensure a coordinated approach to care.

  • Collaborate with other healthcare professionals to ensure holistic care for each resident.

Required Qualifications

  • Bachelorโ€™s Degree in Social Work or a related field (Masterโ€™s preferred).

  • Previous experience in a healthcare or rehabilitation setting is preferred.

  • Strong interpersonal and communication skills, with the ability to build rapport with residents, families, and colleagues.
