This role will be performed remotely, allowing for flexibility in your work hours while providing the support and services our patients need. You will collaborate with a multidisciplinary team through virtual channels to engage in integrated care delivery. As a Licensed Clinical Social Worker, you will report to the Social Work Manager and undertake several essential duties:

• Evaluate diagnoses and address a range of psychosocial issues using tailored clinical social work skills, assisting both patients and their families in overcoming personal and environmental challenges.
• Adhere to professional standards as outlined by the NASW Code of Ethics while consistently applying our organization’s values in every interaction.
• Conduct bio-psychosocial evaluations, aiming to identify and eliminate barriers to treatment and enhance the patient’s overall health status.
• Collaborate with medical and support staff by providing insights into the patient’s psychosocial background and needs, aiding in a comprehensive care programming that promotes treatment compliance.
• Develop personalized treatment plans, including both individual and group therapy modalities, while analyzing treatment effectiveness to optimize health outcomes.
• Document all social work activities accurately within patients' medical records in compliance with departmental standards.
• Educate patients and families on navigating medical recommendations and social work interventions designed to restore optimal functioning.
• Assess advocacy needs and coordinate community resources, emphasizing self-determination in improving social support for clients.
• Maintain detailed case-related statistics and contribute to reports in adherence with departmental protocols.
• Engage in continuous professional development through educational activities and training relevant to clinical responsibilities.
• Uphold department policies and procedures to foster a safe environment of care while remaining compliant with all applicable laws and regulations.

The ideal candidate will possess the following qualifications:

• A Master’s Degree in Social Work from an accredited institution.
• A current Licensed Clinical Social Worker (LCSW) certification from the State Board of Social Work Licensure.
• Demonstrated knowledge of human development, aging processes, cultural considerations, and psychosocial theories relevant to the care of diverse populations.

Preferred candidates will have between six months to one year of clinical experience in health or mental health settings, particularly with populations requiring support in hospice care. Familiarity with specific needs across age groups and cultural patterns within the community is highly valuable for this position.
