You will work collaboratively within an interdisciplinary team, including psychiatrists and other healthcare professionals, to ensure comprehensive care for our patients.

• Conduct thorough biopsychosocial assessments to evaluate clients' needs and develop personalized treatment plans.
• Provide individual, family, and group therapy using evidence-based practices such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Eye Movement Desensitization and Reprocessing (EMDR).
• Implement crisis interventions and safety planning for clients at risk, ensuring immediate support and resources are available.
• Work collaboratively with healthcare professionals to foster a coordinated approach to treatment, breaking down barriers to effective communication.
• Guide clients in resource navigation, helping them access vital services such as housing assistance, financial support, and community resources.
• Maintain meticulous and confidential clinical documentation, adhering to HIPAA standards to protect patient privacy.
• Participate in regular case consultations and team meetings, contributing insights to enhance client care strategies.

You will work primarily with individuals and families dealing with the emotional impact of terminal illness and transitions related to hospice care. This role requires a high level of empathy, as you will support clients facing significant life challenges while helping them maintain their mental well-being.

• Active Licensed Clinical Social Worker (LCSW) license issued by the state of Oregon.
• Master’s Degree in Social Work (MSW) from an accredited institution.
• A minimum of 2 years of clinical experience, preferably in a mental health or hospice setting.
• Strong foundation in trauma-informed care and therapeutic modalities.
• Proficiency in electronic health record (EHR) systems for efficient documentation and communication.
• Excellent interpersonal, communications, and organizational skills to effectively collaborate with clients and colleagues.

• Experience with case management and crisis intervention in a counseling setting.
• Familiarity with community resources related to hospice care and mental health.
• Demonstrated ability to lead group sessions and facilitate family therapy.
