Work Environment

This position functions in a hybrid remote setting. You will conduct sessions both via video chat and through outpatient appointments, allowing you to maintain flexibility while engaging with clients effectively. The majority of your interactions will focus on providing compassionate support to patients and their families as they navigate complicated emotions and decisions associated with hospice care.

Key Responsibilities
  • Conduct thorough initial assessments of clients' mental health needs, employing empathetic listening and clinical expertise.
  • Develop and implement individualized treatment plans that reflect each clientโ€™s goals, preferences, and unique situations.
  • Facilitate individual counseling sessions via video conferencing, ensuring effective communication and emotional support.
  • Collaborate with a multidisciplinary team, including nurses and medical staff, to ensure the delivery of comprehensive care.
  • Educate clients and families about available resources, coping strategies, and support networks to enhance their overall quality of life.
  • Engage in advocacy efforts, empowering clients to identify and pursue personal goals related to their emotional and social well-being.
  • Document all client interactions and treatment plans using electronic health record (EHR) systems, maintaining confidentiality and compliance with HIPAA standards.
Patient Population / Client Focus

This role involves working closely with adult clients in the hospice care environment, many of whom may be dealing with terminal illnesses and the accompanying emotional complexities. You will play a crucial role in supporting grieving families, helping them navigate the emotional landscape of loss, and fostering resilience during difficult times.

Qualifications
  • Masterโ€™s degree in Social Work from an accredited institution.
  • Active and unrestricted Licensed Clinical Social Worker (LCSW) credentials with no disciplinary actions or ethical violations.
  • Proven expertise in conducting mental health assessments, diagnoses, and treatment planning within a clinical framework.
  • Excellent communication and interpersonal skills to build rapport with clients from diverse backgrounds.
  • Proficiency in using technology for remote therapy sessions, including familiarity with various videoconferencing platforms.
Preferred Experience
  • Previous experience working in hospice, palliative care, or related settings is highly desirable.

Compensation and Benefits

You will receive a competitive annual salary ranging from USD 115,000 to USD 130,000, commensurate with your experience and qualifications.
