Work Environment

This role operates within a hospice care environment, where the blend of clinical expertise and compassion is paramount. Key Responsibilities
  • Provide direct clinical services to clients, including assessment, individual therapy sessions, and group therapy for those dealing with severe emotional disorders and major mental health challenges.
  • Develop integrated care plans that address the unique needs of each client and facilitate access to necessary services and support systems.
  • Participate collaboratively with a multi-disciplinary team to create a cohesive approach in advancing patient care and outcomes, striving to meet common goals.
  • Conduct evidence-based screenings and participate in ongoing assessments to adapt care plans based on client progress and emerging needs.
  • Actively monitor client progress throughout therapy, ensuring effective communication and timely documentation of meetings, treatment plans, and discharge criteria.
  • Tackle crisis situations effectively, employing sound judgment to ensure clients receive timely and appropriate interventions.
  • Engage in continuous professional development, including attending training and workshops to enhance skills in dealing with diverse populations.
  • Serve as a resource for families and caregivers, providing guidance and education concerning the emotional and psychological aspects of caring for clients in hospice care.
  • Participate in quality improvement committees aimed at enhancing agency service delivery, implementing strategies that foster higher quality healthcare standards.
  • Be available for on-call coverage as assigned, ensuring that help is accessible during emergency situations outside standard operating hours.
Patient Population / Client Focus

This position focuses primarily on clients within the hospice care paradigm, including frail elderly individuals and those facing end-of-life challenges. The LCSW must not only possess clinical skills but also embody compassion and empathy, providing emotional support to patients and their families during one of the most difficult times of their lives. Collaboration with medical professionals is crucial to address the holistic needs of clients.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited program is required.
  • Active licensure as a Clinical Social Worker (LCSW) in Texas is mandated.
  • Strong analytical skills with expert knowledge of assessment, diagnosis, treatment methodologies, and crisis management.
  • Excellent verbal and written communication abilities, adept at building rapport with diverse populations.
