Patient Care Coordinator

Key Responsibilities
  • Oversee and coordinate care for approximately 40 patients and their families
  • Maintain strong, professional relationships with patients, families, caregivers, and internal teams
  • Review and manage updates that may have occurred outside standard office hours
  • Ensure all patient records, case notes, and communications are accurately updated and maintained
  • Identify communication gaps and help improve processes and patient experiences
  • Coordinate and support new patient cases and caregiver placements
  • Work closely with HR and operations teams regarding caregiver onboarding and case management
  • Monitor ongoing patient situations and proactively escalate concerns when necessary
  • Handle sensitive conversations with professionalism, empathy, and composure
  • Support continuous improvement initiatives and contribute creative ideas and solutions
  • Learn and understand the company's full care coordination systems and protocols
Requirements
  • Previous experience in patient care, healthcare support, care coordination, homecare, medical administration, or a similar people-focused environment
  • Excellent verbal and written English communication skills
  • Well-spoken, professional, and confident interacting with high-level individuals and families
  • Strong emotional intelligence and empathy
  • Ability to remain calm and professional in high-pressure or emotionally charged situations
