Case Management Assistant

Location: Fully remote- MUST live in California!

MUST have EPIC experience

We're seeking an experienced Case Management Assistant II to support a busy Care Coordination team within a leading healthcare organization. This is a non-clinical position focused on discharge planning, transition-of-care coordination, and skilled nursing facility admissions and intake.

Responsibilities
  • Coordinate transition-of-care and discharge-related activities
  • Assist with skilled nursing facility admissions and intake processes
  • Communicate with healthcare providers, facilities, families, insurance contacts, and external vendors
  • Gather, organize, and maintain required documentation and records
  • Support timely patient placement and discharge efforts
  • Navigate multiple priorities in a fast-paced environment
  • Follow established workflows and escalate questions when appropriate
  • Work independently while collaborating effectively with cross-functional teams
Required Qualifications
  • High school diploma, GED, or equivalent
  • EPIC experience is required
  • At least 1 year of recent relevant healthcare experience
  • Experience supporting healthcare operations, care coordination, or related administrative functions
  • Strong computer skills, including word processing and electronic documentation
  • Excellent verbal and written communication skills
  • Ability to work independently with minimal supervision
  • Strong organizational, prioritization, and multitasking abilities
  • Ability to exercise sound judgment and seek guidance when needed
Preferred Qualifications
  • Experience with skilled nursing facility admissions or intake
  • Experience working in a skilled nursing facility or post-acute care environment
  • Experience with insurance workflows or payer coordination
  • Experience communicating with healthcare providers, facilities, families, and outside vendors

Job Type & Location: This is a Contract position based out of Mather, CA.

Pay and Benefits: The pay range for this position is $24.00 - $24.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment. Benefits are subject to change and may be subject to specific elections, plan, or program terms.
