  • Provide ongoing support and therapeutic interventions, assisting clients in navigating complex emotional challenges often associated with end-of-life situations.
  • Maintain accurate and up-to-date electronic medical records that reflect clients' progress and any changes in their condition, utilizing our custom electronic health record (EHR) system.
  • Participate in interdisciplinary team meetings to contribute insights and collaboratively refine patient care strategies.
  • Stay informed of new trends and updates in the field of social work and become familiar with hospice care best practices to enhance patient outcomes.
Patient Population / Client Focus

As an LCSW at Warriors Heart, you will primarily work with terminally ill patients and their families within a hospice setting. Your role will be instrumental in addressing the emotional and psychological complexities that arise during end-of-life care. This includes offering bereavement support, navigating family dynamics, and facilitating coping strategies in collaboration with medical staff and family members.

Qualifications
  • A Master’s degree in Social Work from an accredited program.
