Client: Tennessee Department of Health (TDH) - Division of Family Health & Wellness
Location: Remote within Tennessee (Monthly onsite meetings in Nashville required)
Duration: 07/20/2026 - 06/30/2027
Work Schedule: Monday-Friday | 8:00 AM - 4:30 PM | 37.5 hrs/week
Preference: Tennessee Residents Preferred
Overview
The Tennessee Department of Health is seeking a Public Health Program Director to lead the expansion of the CHANT Welcome Home initiative under the CMS-funded Rural Health Transformation Program. This role will oversee maternal and infant health programs in rural communities, manage grant-funded contracts, coordinate software implementation efforts, and collaborate with community partners to improve health outcomes across maternity care desert counties.
Key Responsibilities
  • Lead and oversee the Welcome Home Kit and enhanced care coordination initiatives across rural Tennessee.
  • Manage grant-funded contracts, budgets, deliverables, expenditures, and compliance activities.
  • Serve as the primary liaison for grantees, vendors, and community partners.
  • Monitor program performance through data analysis, reporting, and evaluation activities.
  • Collaborate with epidemiologists and public health teams on program evaluation strategies.
  • Support procurement and implementation of call center and case management software platforms.
  • Develop sustainability strategies and identify future funding opportunities.
  • Conduct site visits, stakeholder meetings, and statewide partner engagement activities.
  • Coordinate with internal and external partners to advance maternal, fetal, and infant health priorities.
Required Qualifications
  • Bachelor's degree in Public Health or related field.
  • 5+ years of progressively responsible public health program experience.
  • Strong experience in grant management, contract oversight, budgeting, and reporting.
  • Experience leading multi-stakeholder initiatives and community partnerships.
  • Ability to analyze public health data and apply findings to program planning and evaluation.
  • Excellent project management, organizational, communication, and stakeholder engagement skills.
Preferred Qualifications
  • Experience providing training and technical assistance.
  • Knowledge of maternal, fetal, infant health, and rural health systems.
  • Experience working with community-based organizations and healthcare programs.
Additional Information
  • First day onsite in Nashville required for onboarding and equipment deployment.
  • Monthly onsite attendance may be required by management.
  • Travel to Nashville for routine meetings is not reimbursable.
  • Statewide travel may be required for program and stakeholder engagement activities.
