Mohawk Trail Counseling Center is seeking a dedicated and compassionate Remote Licensed Professional Counselor (LPC) to join our expanding team. In this full-time position based in Gresham, Oregon, the LPC will focus on providing essential therapeutic interventions and support to individuals dealing with various mental health challenges. With a competitive salary range of USD 87,000 – 105,000 per year, this role is instrumental in fostering the emotional, physical, and mental growth of our clients.

Located in Gresham, Oregon, Mohawk Trail Counseling Center is a multifaceted clinic that offers a wide range of services, including obstetrics, pediatrics, general medicine, family planning, and dentistry. Our mission is to address and manage the various social issues impacting our community through comprehensive education, intervention, and management strategies. This role offers the flexibility of a remote work setting, allowing counselors to conduct sessions from their home or any suitable environment. While our workforce operates primarily through telemedicine platforms, our outpatient office in Gresham serves as a hub for team meetings and administrative tasks. The collaborative atmosphere encourages open communication and teamwork among colleagues while adhering to agency and Federally Qualified Health Center (FQHC) guidelines.

The Remote LPC will play a crucial role within the primary care team and will be responsible for the following:

• Assisting primary care providers in identifying, treating, and managing mental health and psychosocial issues, contributing to holistic patient care.
• Conducting thorough patient intakes, including diagnostic and functional evaluations, and recommending personalized treatment plans to primary care providers.
• Providing focused therapeutic interventions for patients in need of mental health support, ensuring efficient and effective care delivery.
• Collaborating with primary care providers to determine the appropriateness of mental health services for patients, referring them to specialized services when necessary.
• Following up with patients to monitor their progress, adjusting treatment plans as needed to align with their shifting needs.
• Developing relapse prevention strategies to help patients maintain stability and improve their overall functioning.
• Detecting 'at-risk' patients and collaborating on prevention plans to avert deterioration of their mental health conditions.
• Coordinating the delivery of behavioral health services, ensuring a seamless experience for patients.
• Documenting patient progress and clinical assessments in treatment charts, maintaining accurate and complete records in a timely manner.
• Working in concert with healthcare providers to establish comprehensive treatment plans that address both medical and psychological needs.
• Providing patients with self-management skills and educational resources to empower them in their recovery journey.

The LPC will primarily serve a diverse patient population with varied mental health needs, including anxiety disorders, depression, trauma responses, and other psychosocial issues. By offering individualized attention and culturally sensitive interventions, we ensure that our clients feel heard and supported. Those with complex medical and psychological conditions will be addressed with tailored care that respects their unique circumstances.

To be successful in this role, candidates must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling.
• Current LPC (Licensed Professional Counselor) License valid in Oregon.
• Strong foundational knowledge in mental health assessment, treatment planning, developmental psychopathology, and therapeutic techniques, including family and group therapy.
• Proficient understanding of the DSM-IV diagnoses and behavioral therapy methodologies.
• Excellent communication skills with an emphasis on building effective, cooperative relationships with patients, families, and interdisciplinary team members.
• Ability to assess patient needs accurately and evaluate the effectiveness of care provided.
• Capacity to maintain clinical records and prepare necessary reports in compliance with legal and ethical standards.

While not mandatory, preferred qualifications include:

• Bilingual proficiency in English and Spanish, enhancing communication with a wider range of clients.
• A solid understanding of community resources available to support clients in their treatment and recovery.
• Experience adhering to the Code of Ethical Conduct for Clinical Professionals.

This position offers a competitive annual salary ranging from USD 87,000 to 105,000 based on experience and qualifications.
