Key Responsibilities

As a Licensed Professional Counselor at Mohawk Trail Counseling Center, your day-to-day responsibilities will include:

• Conducting comprehensive patient intakes that involve detailed diagnostic and functional evaluations to inform treatment plans.
• Collaborating with primary care providers to identify and address mental health and psychosocial issues, ensuring an integrated approach to patient care.
• Implementing brief, focused therapeutic interventions tailored to meet the immediate needs of patients seeking mental health support.
• Continuously monitoring patient progress through follow-up sessions, and making recommendations for any needed adjustments in treatment strategy.
• Developing and documenting relapse prevention plans for patients identified as at risk, facilitating ongoing support for stable functioning.
• Linking patients to additional community resources and treatment providers with their consent, ensuring comprehensive care coordination.
• Maintaining accurate clinical records and progress notes in compliance with regulatory standards and agency policies.
• Providing consultation with clinic management and other team members to enhance behavioral services and evaluate program outcomes effectively.

At Mohawk Trail Counseling Center, you will serve a diverse patient population, addressing a variety of mental health concerns including anxiety, depression, adjustment disorders, and more complicated psychosocial conditions. Qualifications

To excel in this role, candidates must possess:

• A Master’s Degree in Clinical Counseling or a related field.
• A current LPC (Licensed Professional Counselor) license, ensuring compliance with state regulations.

• Knowledge of DSM-IV diagnoses and behavioral therapy practices.

Preferred Experience
